  • From a client’s GP if I need to get a referral about a client’s medical condition before doing a treatment with them.

 

  • If client is paying Online via a Checkout Payment Gateway, client’s personal and some of their payment details will be shown. (Clients full payment details will not be shown.)

How long we keep information

For information on how long we keep personal information, see our retention schedule at eu.jotform.com.  Your information will be kept secure until I delete it.  Data will be kept for up to 7 years.

This data processor does the following activities for us: SquareUp is a point-of-sale payment gateway system which i use. it enables a way for my business to accept online card payments from customers paying for treatments on my website. Or by accepting payments on my squareup portable card reader as a payment processing for contactless, chip and pin, or google and apple Pay. It also acts a payment invoice system, customer booking and appointments, and customer loyality programs.

Duty of confidentiality

We are subject to a common law duty of confidentiality. However, there are circumstances where we will share relevant health and care information. These are where:

·        you’ve provided us with your consent (we have taken it as implied to provide you with care, or you have given it explicitly for other uses);

·        we have a legal requirement (including court orders) to collect, share or use the data;

·        on a case-by-case basis, the public interest to collect, share and use the data overrides the public interest served by protecting the duty of confidentiality (for example sharing information with the police to support the detection or prevention of serious crime);
